Humana - - Responsibilities: Provide direct patient care under the direction of the RN; Assist with medication management and monitoring; Monitor and report vital signs; Maintain personal hygiene and assist with activities of daily living; Assist with feeding, nutrition, and meal positioning; Assist with toileting and ostomy care; Support mobility, transfers, and ambulation per Plan of Care; Document care provided and communications with patients and staff; Maintain infection control standards and a clean environment; Assist with light household duties when needed
