Patient Care Coordinator

Palo Verde is seeking a Patient Care Coordinator for our Outpatient Therapy Services Department. Come join our team. The Patient Care Coordinator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient Services as part of their continuum of care treatment. This position will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. They will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. This position frequently supports the Outpatient Programming and Therapists as directed by Outpatient Leadership.

Palo Verde Behavioral Health Hospital provides comprehensive behavioral healthcare for children, adolescents, adults, and senior adults suffering from mental health and addictive illnesses. PVBH is an 84 bed, short-term, acute care hospital offering inpatient, partial and intensive outpatient hospitalization, as well as specialized treatment programs for chemical dependency. PVBH has an excellent reputation as an innovative hospital with caring, dedicated staff, experienced in the delivery of behavioral healthcare. Situated in Tucson, AZ, at the base of the Catalina Foothills, PVBH is minutes to Saguaro National Park and the University of Arizona.

Qualifications

Education: Some college in mental health/behavioral sciences. Preferred: Bachelor's degree from an accredited college or university in social work, psychology, mental health or a related field.

Experience: A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting (preferred).

Additional Requirements: CPR and HWC training as outlined in New Hire Policy must be completed before providing direct care services.
